Privacy Policy

This Privacy Policy explains how Onda Health, LLC(“Onda,” “we,” “us,” or “our”) collects, uses, and shares information when you visit onda.health, contact us, or ask us to check your eligibility for equipment and supplies.

Medical records are handled separately. Onda is a Medicare-enrolled supplier of durable medical equipment. Health information we create or receive to provide and bill for your equipment is protected health information (PHI) governed by our HIPAA Notice of Privacy Practices, not this policy. This Privacy Policy covers the information you share through our website, forms, phone, and marketing.

Information we collect

Information you give us

  • Contact details — name, phone number, email, and mailing address.
  • Eligibility details— date of birth, insurance or Medicare information, your doctor's information, and the condition or equipment you're asking about, when you request an eligibility check or ask us to coordinate care.
  • Messages — anything you tell us in a form, email, text, or phone call.

Information we collect automatically

  • Device and usage data — IP address, browser type, pages viewed, and how you got to our site.
  • Cookies and similar technologies — including analytics and advertising technologies described below.

Information from other sources

  • Marketing and lead-generation partnerswho share your contact information with us after you've asked to learn more about medical equipment, along with the consent they collected on our behalf.
  • Your insurer or Medicare, to verify coverage, and your healthcare providers, to coordinate your prescription.

How we use your information

  • Verify your insurance and check whether you're eligible for equipment.
  • Coordinate your prescription and shipments with your doctor and insurer.
  • Respond to you, provide support, and send service and resupply updates.
  • Send you marketing messages where you've agreed to receive them.
  • Measure and improve our website and advertising.
  • Protect against fraud and misuse, and comply with the law.

Cookies, analytics, and advertising

We use cookies and similar technologies for two purposes: analytics (to understand how the site is used) and advertising (to show you relevant ads on other platforms and measure whether they work). Our advertising partners may set their own cookies or receive identifiers through pixels on our site.

You can control cookies through your browser settings. Because some of this activity may be considered a “sale” or “sharing” of personal information under state law, you also have the choices described in Your privacy rights below.

How we share information

We do not sell your information for money. We share it in these ways:

  • Service providers who work on our behalf — hosting, communications, insurance verification, and analytics — under contracts that limit their use of it.
  • Advertising partners, through the cookies and pixels described above. This may be considered “sharing” or a “sale” under some state laws.
  • Your insurer, Medicare, and your healthcare providers, to verify coverage and coordinate your care.
  • Legal and safety— when required by law, to enforce our terms, or to protect people's rights and safety.
  • Business transfers — in connection with a merger, financing, or sale of assets.

We do not share information you send us by text message, or your consent to receive texts, with third parties or affiliates for their own marketing.

Your privacy rights

Depending on where you live, you may have some or all of the following rights regarding the personal information we hold about you:

How to exercise your rights. Email info@onda.health or call 855-438-1998. We'll verify your identity before acting on your request, and you may use an authorized agent. We won't discriminate against you for exercising these rights.

Do Not Sell or Share / Global Privacy Control.To opt out of the sale or sharing of your personal information for advertising, email us at the address above with “Do Not Sell or Share” in the subject line. Where required, we also honor the Global Privacy Control browser signal as a valid opt-out.

California residents

In the past 12 months we may have collected identifiers, contact and commercial information, internet activity, and — with your request — health-related eligibility information (treated as sensitive personal information). We use it for the purposes above and do not use sensitive information to infer characteristics about you. California residents have the rights listed above under the CCPA/CPRA.

Consumer health data

Some states (such as Washington and Nevada) give you additional rights over health-related information collected outside of HIPAA. If you've shared eligibility details with us through the website, you may ask us to access or delete that information using the contact details above.

Data retention

We keep personal information for as long as needed to provide our services, meet legal, tax, and recordkeeping requirements, and resolve disputes — then delete or de-identify it. PHI is retained as described in the HIPAA notice.

Security

We use reasonable administrative, technical, and physical safeguards to protect your information. No system is perfectly secure, so we can't guarantee absolute security.

Children's privacy

Our website and services are intended for adults. We don't knowingly collect personal information from children under 18. If you believe a child has given us information, contact us and we'll delete it.

Other sites

Our site may link to third-party sites we don't control. Their privacy practices are their own — please review their policies.

Changes to this policy

We may update this policy from time to time. We'll change the “Last updated” date above, and significant changes will be posted on this page.

Text Message (SMS) Terms

These terms govern the text messages you exchange with Onda Health, LLC. By giving us your mobile number and agreeing to receive texts, you accept these terms.

What we text you about

  • Order and account updates — eligibility, shipping, resupply reminders, and coverage or cost changes.
  • Getting started — messages to help us qualify your request and set up insurance verification, including messages sent through automated systems.
  • Occasional promotions— offers and news about our services, where you've agreed to receive them.

Your consent

We only text you after you provide your prior express written consent — for example, by checking a consent box on our form or texting us first. Consent is not a condition of purchase. Your consent applies only to Onda; we don't share your number or your opt-in with anyone else for their marketing.

Message frequency & cost

Message frequency varies based on your interactions with us. Message and data rates may apply — check with your mobile carrier. Onda does not charge for the messages themselves.

How to stop or get help

Reply STOPto any message to unsubscribe; we'll send one confirmation and stop texting you. Reply HELP for help, or contact us at info@onda.health or 855-438-1998. Opting out of texts doesn't remove you from phone or email contact about your orders.

Carriers & delivery

Message delivery is subject to your carrier and isn't guaranteed. Mobile carriers are not liable for delayed or undelivered messages.

Privacy

Information you share by text is handled under our Privacy Policy. See the HIPAA notice for how we protect health information.

HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Onda Health, LLC is required by law to protect the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your health information

For treatment

We use and share your PHI to provide and coordinate your equipment and supplies — for example, working with your physician on your prescription and setup.

For payment

We use and share your PHI to verify coverage and bill and collect from Medicare, Medicaid, or your insurer — for example, submitting claims and obtaining prior authorization.

For healthcare operations

We use your PHI to run our business — for example, quality review, training, and contacting you about resupply and service.

Other uses and disclosures permitted or required by law

We may use or disclose your PHI without your authorization when the law allows, including:

  • When required by federal, state, or local law.
  • For public health and safety activities, and to report certain issues about products.
  • For health oversight, judicial and law-enforcement requests, and to avert a serious threat to health or safety.
  • For workers' compensation, research under safeguards, organ donation, coroners, and specialized government functions.
  • To business associates who perform services for us under contracts that require them to protect your PHI.

To a family member, friend, or other person you involve in your care — but only the information relevant to that person's involvement, and only if you don't object.

Uses that require your written authorization

Most uses and disclosures not described above will be made only with your written authorization, which you may revoke at any time. This includes most marketing, any sale of your PHI, and disclosure of psychotherapy notes.

Your rights regarding your health information

  • Access — inspect and get a copy of your PHI, usually within 30 days.
  • Amend — ask us to correct information you believe is wrong or incomplete.
  • Accounting of disclosures— get a list of certain disclosures we've made.
  • Request restrictions — ask us to limit how we use or share your PHI. You may also require us not to share information about a service with your health plan if you paid for it in full out of pocket.
  • Confidential communications — ask us to contact you a certain way or at a certain place.
  • Paper copy — get a paper copy of this notice, even if you agreed to receive it electronically.
  • Breach notification— be notified if there's a breach of your unsecured PHI.

Our duties

We are required to maintain the privacy of your PHI and to notify you following a breach. We must follow the notice in effect. We reserve the right to change this notice and make the new terms apply to all PHI we maintain; a current copy will always be available on this page.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

The site is intended for adults in the United States. You agree to give accurate information when you contact us or request an eligibility check.

The site and its content — text, design, logos, and the “Onda” name and marks — belong to Onda Health, LLC or our licensors. We grant you a limited, personal, non-commercial license to view it. Don't copy, resell, or reuse it without our permission.

Acceptable use

  • Don't use the site unlawfully or to infringe others' rights.
  • Don't attempt to disrupt, hack, or gain unauthorized access to the site.
  • Don't submit false information or impersonate anyone.

Third-party links

The site may link to sites we don't operate. We aren't responsible for their content or practices.

Governing law & disputes

These terms are governed by the laws of the State of Utah, without regard to its conflict-of-laws rules. Any dispute will be resolved in the state or federal courts located in Salt Lake County, Utah, and you consent to their jurisdiction — except where applicable law gives you the right to bring a claim elsewhere.

Changes

We may update these terms from time to time. Continued use of the site after we post changes means you accept them.
